GPM, Corp is thrilled to announce the 2019 LTC Summit at Biltmore! Sunday, September 29th – Tuesday, October 1st at the Biltmore Estate in Asheville, North Carolina.

The LTC Summit, and CMS’ Patient Driven Payment Model for SNFs, both kick-off in October 2019. What an opportune date to explore this year’s meeting theme – aligning medical practices and LTPAC Facilities to achieve better outcomes and share risk-based payments.

The advent of PDPM requires Practitioners and SNFs align their documentation – starting at Admissions. Until now, the value-based payment programs for Facilities, and Practices, measured performance for the same patient with different metrics. Beginning October 1st, all payment models use ICD-10 diagnoses as the foundation; Practitioners alone assign those diagnoses.

Throughout this conference, you’ll learn how innovators are re-imagining the PAC-LTC service industry as one integrated CareTeam. One where an individual patient, their family, and providers, traverse multiple payment programs safely– BPCI-A, PDPM, MIPS, MSSP, PCF, HHA-PPS, and I-SNP.  Each program was created by CMS regulation but have different, and occasionally divergent, metrics. Our clinicians are responsible, but it’s only through technology they can become accountable! This requires a virtual CareTeam that integrates Practitioners, Medical Directors, the facility staff, pharmacists, and care managers.

We want the LTC summit to be a unique gathering, one which integrates – policy insights, practical strategies for better performance, and technological innovation. Our goal is to systematically improve your ability to transition through stages to full risk-bearing payment models.

Networking Mixer

On Monday, September 30th there will be a networking event. You will receive two (2) drink tickets for use at the networking mixer.

Biltmore offers Complimentary Guest Internet access throughout the estate, including event spaces. Further information will be provided after ticket purchase.

Attendee Registration Policies

Substitution Policy: If you register and then are unable to attend, you may send someone in your place at no additional charge. Substitutions must be submitted in writing by August 30th, 2019. After this date, all substitutions will be processed at on-site registration. Please e-mail mgroppe@gpm.md the full name, address, telephone number and e-mail address of the individual who will be replacing you, thereby authorizing the substitution to be made.

Split/Share Registration Policy: To attend the conference all attendees must be registered, and the registration cannot be transferred or shared with anyone else for the entire conference.

Cancellation Policy: Please submit all cancellation requests in writing by e-mail to mgroppe@gpm.md by August 30th, 2019. Your cancellation will be acknowledged by email. If you do not receive an acknowledgment within two weeks of sending in your cancellation, please contact Matthew Groppe at 828-348-1264 to verify receipt. Cancellations will be processed according to the following schedule/received by:
